MOTS-c and SS-31: The Mitochondrial Pair

One is a signal your mitochondria send. The other protects the membrane they run on. They do different jobs.

Why mitochondria come up at all

Mitochondria are the parts of your cells that turn food and oxygen into usable energy. When they work poorly, the result is not a specific disease so much as a general one: fatigue, poor endurance, worse glucose handling, slower recovery.

They are also unusual. Mitochondria carry their own DNA, separate from the DNA in the cell nucleus — a leftover from an ancient point when they were free-living bacteria that got absorbed into larger cells.

That detail is the reason MOTS-c exists as a thing to research at all.

MOTS-c: a signal from inside the mitochondria

MOTS-c is not a drug that was designed. It is a peptide encoded in mitochondrial DNA itself, discovered when researchers looked more carefully at a genome everyone assumed was fully mapped.

Changhan Lee and Pinchas Cohen at the USC Leonard Davis School of Gerontology described it in *Free Radical Biology and Medicine* in 2016 as a mitochondrial-derived peptide regulating muscle and fat metabolism.

What makes it interesting is the direction of the message. Mitochondria are normally thought of as receiving instructions from the cell. MOTS-c suggests they send them too — that when energy demand is high, the mitochondria signal outward to change how muscle and fat tissue behave. It is associated with AMPK, the cellular pathway that switches on when energy runs low, which is the same pathway exercise and fasting act through.

That is why it comes up in endurance and metabolic research rather than muscle-building research. It is a signal about energy availability, not a growth signal.

SS-31: protecting the surface everything runs on

SS-31 does something completely different, and confusing the two is the most common mistake made about this pair.

Inside every mitochondrion is a heavily folded inner membrane. That membrane is where energy production physically happens, and it is built around a distinctive fat called cardiolipin. Cardiolipin holds the folds in shape and organises the energy-producing machinery into working clusters.

When cardiolipin is damaged — which happens with age, and in a long list of conditions involving energy failure — the structure degrades and output falls.

Hazel Szeto at Weill Cornell described SS-31 in the *British Journal of Pharmacology* in 2014 as the first compound of its kind to protect cardiolipin specifically, with the aim of restoring mitochondrial energy production rather than substituting for it.

So the division of labour is clean. MOTS-c is a message about energy. SS-31 protects the hardware that produces it. Neither does the other's job, which is precisely why they are researched as a pair in the Mitochondrial Powerhouse Protocol.

How strong is the evidence

Stronger than most peptides on this site, and still not strong.

SS-31 has been through genuine human clinical trials under the name elamipretide, in conditions including primary mitochondrial myopathy and Barth syndrome. Those trials exist, are independent, and are published — which puts it well ahead of most of this field. The results have been mixed, with some endpoints met and others missed.

MOTS-c is earlier. Most of the work is in cells and animals, and human data is largely observational — measuring natural MOTS-c levels in people and noting how they relate to fitness or metabolic health, rather than administering it and measuring what happens.

Neither compound is approved as a medicine. Neither has long-term safety data in healthy adults. And almost all of the research has been conducted in the context of disease or ageing, not in healthy people looking for an edge — a gap worth keeping in mind whenever results get quoted in that second context.
